Early Alert Form

The Early Alert Form is a retention and student support initiative designed to identify students who may be at risk for academic difficulty or failure, or may be having behavioral or emotional difficulties at St. Michael’s. This form allows faculty, staff, coaches, or advisors to alert the counseling and retention offices to any student who is not making satisfactory progress and/or is exhibiting behaviors that may lead to academic difficulty or departure from St. Michael’s.

Once a student is identified via the form below, the appropriate office and/or support service personnel will attempt to contact and work with the student and/or the parents in an effort to improve the student’s performance by encouraging participation in student programs, tutorials, counseling, and/or workshops that may help the student academically or personally. Observations should be objectively stated.

It should be noted that Early Alerts are not grades, nor are they legal documents. There are no permanent records of Early Alerts and they are not tied to a student’s transcript or academic record. The Early Alert form is a mechanism to inform St. Michael’s staff on how to help students succeed and ultimately, retain students at St. Michael’s.
